Teen Lodge Form

This form contains all the information we need to enroll your child in the Teen Lodge. Before you begin please be sure to have the following documents scanned and ready to upload into this form. Make a separate scan for each page of the forms as they will be uploaded separately.

1. Front and back copy of the student's insurance card

2. Completed Health History Form

3. Signed Student Liability Release Form

4. A recent photo of the student clearly showing their face (such as a recent school photo)

The information below is very important as you are making travel plans and preparing to stay in the Teen lodge. All of this information can also be found at the program websites on the Teen Lodge page.

If the teen is traveling with his/her parents, please be sure to arrive in Beaver Creek by 2:45 pm on Friday, June 28th for the 3:00 welcome meeting. If flying in and out of the Denver airport, be sure to allow 2 hours at the Denver airport for check in and security. Really!

Travel Teen Only: If the teen is traveling alone into the Denver airport, we can arrange to meet the student at the airport and make sure they catch the correct Summit Express shuttle service. We will have someone waiting to meet them when they get off the shuttle in Beaver Creek. On their return home, we can arrange to have them catch the Summit Express shuttle service from Beaver Creek to the airport, meet them at the airport as they get off the shuttle, make sure they check into the proper airline and get into the security line at the airport. The family of the student is required to make all air travel and shuttle arrangements with Summit Express shuttle service. Call 970-668-6000 or visit www.summitexpress.com and mention code “CSI2021” to receive a 10% discount. Colorado Vocal Institute will not arrange or pay for any of the student’s air travel or Summit Express shuttle travel. In order to take advantage of this Teen Travel service, you must submit a Teen Travel Form and pay the $200 fee so we can arrange when and where to meet your teen and make sure they get to the right place!

One of the teen counselors will meet the students at the Teen Lodging Check in desk in Beaver Creek at 2:45 pm on Friday June 18th. Be sure to arrange your travel so that you arrive at the check in desk between 2:45 and 3:00 for the Institute meeting at 3:00. If this is not possible due to airlines schedules, contact the institute office to arrange arrival. Also remember that we are 2 hours away from the Denver airport as you make your travel arrangements. Students can keep their luggage with them for the 3:00 meeting. After the meeting they take a short walk to the teen lodge and check into their rooms. At check out, each student will inspect the condition of the room along with a Teen counselor and Beaver Creek Lodging staff. Any damages incurred during the student’s stay will be the responsibility of the students.

Check out is after breakfast on Saturday, June 26th. Also remember that we are over 3 hours away from the Denver airport as you make your travel arrangements. You can place your luggage in your counselors’ rooms until after the concerts are over if needed. At check out, each student will inspect the condition of the room along with a CSI Teen counselor and Beaver Creek staff. Any damages incurred during the student’s stay will be the responsibility of the students.

June weather in the mountains of Colorado can range from the 30’s to the 80’s. Packing layered clothing is the safest bet in order to keep warm in the evenings and mornings, and cool in the afternoon sunshine. There is also always a chance of a daily shower. Make sure to include concert clothes as well. See the Teen Lodge website for a complete list!

If you are too ill to attend class, notify a counselor. If you get ill during class, tell your teacher and then go to the music office. Singers who are ill enough to miss class or rehearsals are expected to remain in their room.

Meals will be provided to each student as part of the program. Meals for Colorado Vocal Institute begin with dinner on Friday, June 18 and will continue through breakfast on Saturday, June 26th.

Students should bring a small amount of spending money.

After the nightly musical events are over, there are scheduled activities such as hikes, movies in the teen lounge, and more. Be sure to bring the appropriate clothing to enjoy these activities.

Both parents and students participating in the Teen Lodge program are required to read and sign the Code of Conduct and complete the Health Form in order to attend.

THE FOLLOWING WILL RESULT IN IMMEDIATE PARENTAL NOTIFICATION REGARDING DISCIPLINARY ACTION AND POSSIBLE DISMISSAL:

1. Boys in girls’ sleeping rooms OR girls in boys’ sleeping rooms

2. Smoking

3. Use of illegal drugs or substances

4. Illicit or wrongful use of legal drugs or substances

5. Violation of curfew

6. Leaving Copper Mountain Resort without a counselor

7. Disruptive behavior

8. Absences from class/truancy

The Director of Music of the Colorado Institute of Music shall have complete discretion as to any dismissal of a student from the Institute. In the event of any such dismissal, the student’s parent or legal guardian understands and agrees that they shall be responsible for any and all costs required to transport the student back to his or her legal address. They further understand and agree that a student’s dismissal from the Institute shall constitute a forfeiture of all tuition, costs, and registration fees paid.
