CVI Scholarship Committee will hold all information in confidence. Please provide a written statement below explaining why a scholarship is necessary to attend. Please include any unusual expenses or circumstances you would like taken into consideration.
Please mail the first two pages of your current year Form 1040 directly to: Colorado Vocal Institute, 3440 Youngfield St. #111 Wheat Ridge, CO 80033. (If possible, please black out social security numbers before you send. These documents well be destroyed as soon as the application is processed.)
